Support Worker - Personal Care & Community Access

We are seeking compassionate, reliable, and dedicated Support Workers to join our growing team. In this role, you will provide high-quality, person-centred support to NDIS participants and clients, empowering them to live independently and achieve their personal goals.
We are a NDIS registered provider delivering disability support services and expanding into aged care (provider registration in progress). We provide person-centred services in the community with a strong focus on safety, dignity and respect.
Key responsibilities Daily living & community support ● Assist clients with routines, community access and appointments ● Social support and companionship ● Meal preparation, light domestic assistance and shopping Personal care (hands-on care) ● Showering, toileting, grooming, dressing and continence support ● Mobility assistance, transfers and safe manual handling ● Support with eating and hydration where required Basic nursing support (where applicable and trained) ● Observe changes in wellbeing and report to the care team ● Record basic observations if trained (e.g., BP/temperature) ● Medication prompting and documentation (as permitted by policy) ● Follow infection control and safety procedures Documentation & compliance ● Complete shift notes/progress notes accurately ● Follow care plans and report incidents/hazards promptly Essential requirements ● NDIS Worker Screening Check (or willingness to obtain) ● Current First Aid & CPR (or willingness to obtain) ● Driver’s licence and reliable vehicle (travel between clients) ● Strong communication, empathy, reliability and professionalism ● Ability to work a rotating roster including some evenings/weekends Highly regarded ● Certificate III/IV in Individual Support (or similar) is highly regarded. Candidates who are currently enrolled or willing to obtain the qualification will also be considered. ● Experience in disability support, aged care, dementia care, or complex needs ● Manual handling/infection control training What we offer ● Flexible shifts and consistent hours for the right candidates ● Supportive team and ongoing training ● Meaningful community-based work ● Sponsorship may be considered for exceptional candidates (eligibility applies).
